Abstract

The success of a twice-weekly fully supervised regimen of streptomycin plus high dosage isoniazid in the chemotherapy of newly diagnosed pulmonary tuberculosis in patients at Madras provided the stimulus for a series of controlled clinical trials with once-weekly regimens (Tuberculosis Chemotherapy Centre, Madras, 1964, 1970, 1973). Our earlier studies demonstrated that such once-weekly regimens were not satisfactory, due to the relative inadequacy of isoniazid in some of the patients. Efforts were therefore made to employ isoniazid in a sustained release form which could be given in substantially higher dosages so that isoniazid inadequacy would be eliminated. Such a preparation was made available by Messrs. Smith and Nephew through the courtesy of the British Medical Research Council. A series of clinicopharmacological and therapeutic trials was conducted as a joint collaborative programme of the Tuberculosis Chemotherapy Centre of the Indian Council of Medical Research, the World Health Organization, the Government of Tamil Nadu and the British Medical Research Council. Dr Ellard and I will be presenting the findings of some of these investigations.
