Abstract

India continuestohavethehighesttuberculosisincidence,accountingforonefifthoftheglobal incidenceand2/3rdofthecasesinsouthEastAsia.TheTBburdenisalsoaugmentedbymultidrug resistanceandHIV.Althoughinadequateandinappropriatetreatmentisresponsiblefordrugresistance, pathogen’sgeneticbackgroundmayalsoplayarole.Theaimofthisstudywastounderstandthe distributionofdifferentgenotypesof Mycobacteriumtuberculosis in theTiruvallur,ruralareainSouth India anditsassociationwithdrugresistance.Atotalof1649 M. tuberculosis isolatesweregenotypedby IS6110RFLPandspoligotyping.Drugsusceptibilitytestingwasdonebyminimuminhibitory concentrationmethod(MIC)onallthesamples.Asreportedearlier,theisolateswithsingleandlow copy IS6110accountedfor66%amongthe1649 M. tuberculosis strains genotyped.Themajority(84%)of our strainsbelongedtotheEastAfricanIndian(EAI)lineage,28.6%toEAI3sublineageand19.5%toEAI5 sublineage.RifampicinandstreptomycinmonoresistancefollowedbyMDR(Multi-DrugResistance, resistancetoatleastrifampicinandisoniazid)[(OR0.2[95%CI0.11-0.46],P < 0.05)] weremorecommon betweenCentralAsian(CAS),TandBeijingcomparedtoEAIlineage.InspiteofthepredominanceofEAI lineage,itsassociationwithdrugresistancewaslowercomparedtotheothergenotypesprevalentin Tiruvallur,SouthIndia
